Exciting opportunity to work within the MDT environment of the Urgent Care Team supporting early hospital discharges and responding to urgent community response referrals to help prevent admissions to hospital.
To work within the Torbay urgent care team with support and supervision from the multi-disciplinary team. To provide an urgent care service to patients to promote faster recovery from illness, prevent unnecessary acute hospital admissions, support timely discharge and maximise independent living. Working towards ensuring the national targets for urgent care response are met by providing timely interventions in line with the 2 hour and 24-hour response times. This post also covers some intermediate care work to help with patient flow.
Main duties of the job
Completing assessments within patients' homes, providing equipment, carrying out observations under the supervision of the occupational therapists, physiotherapists and nurses. The main focus will be around hospital avoidance rapid assessment to prevent hospital admission or facilitate early discharge. We provide a 7-day service covering the hours of 8am to 7pm including weekends.

Planning and organisation
The post-holder will be required to work flexibly to ensure a consistent and robust service; this will include seven-day working across weekend and bank holiday working, or working in other localities as necessary to meet the needs of the service.
To take responsibility for the daily care and support of delegated patients.
Patient and client care
Work proactively with patients and their families to develop personalised care plans which reflect their choices for end-of-life care.
Liaise closely with a range of providers of palliative care service, so that people will receive quality care aligned to the Gold Standards Framework.
Support terminally ill patients and their carers/relatives, ensuring other relevant agencies are involved.
To maintain accurate and contemporaneous clinical records of care given.
To record activity on a clinical database as required.
Assist in the monitoring and protecting of groups and individuals whose health and well-being may be at risk e.g.: vulnerable adults.
To implement treatment and care to patients under the guidance and supervision of a registered professional.
Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.

Ensure that own contribution to service is delivered in a way that meets the diverse population of Southern Devon and adheres to the trust policy on equality and diversity.
To practice the principles and philosophy of individualised person-centred patient care in accordance with Trust Policy, procedures and regulatory bodies' guidelines.
Finance, equipment and other resources
To participate in ordering of specific equipment, in line with instructions and ensuring safe practice.
Participate in ordering and monitoring of stores, equipment and other appropriate tasks.
Undertake statutory and mandatory and other training as requested.

Be responsible for own personal development and to keep up to date with current issues in clinical field, to include participation and attendance at related meetings, mandatory and in-service training days.

To contribute to the supervision of non-registered health care support workers.
To support junior members of staff and students, promoting and monitoring best practice.
Develop and maintain skills required for patients within a community setting in line with appraisal and KSF.

Work with patients and carers to teach and educate them about the early warning signs to prevent crises.
Contribute to the planning, development and implementation of educational programmes for staff, patients and carers.

Qualifications and Training
* Foundation Degree (FdSc in Healthcare Practice (Nursing Associate)
* Trust induction training
* Experience in a Community Setting

* Understand and demonstrate evidence-based practice
* Understanding of when to seek advice and refer to a registered care
* Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role may contribute to service development
