Targeted Lung Health Check Patient Coordinator

Band 4 £26,530 - £29,114 per annum pro rata

An exciting opportunity has arisen at Milton Keynes Hospital NHS Foundation Trust to join The Targeted Lung Health Check team as a Patient Coordinator.
We are looking for someone with excellent communication skills, both verbal and written and who is able to multitask and manage large amounts of data. You will need to have excellent customer service skills as you will be communicating with patients within this role. You should have keen attention to detail and good organisational skills. The successful applicant will be driven and proficient in the use of computer systems. You must be able to work to deadlines and have the ability to work under pressure, be a team player and have an approachable attitude.

The post holder will support the Trust with improvements in data capture within the Targeted Lung health Check (TLHC) programme and need to organise and manage their workload on a day-to-day basis to meet competing demands and deadlines, including an ability to respond to urgent requests, whilst supporting the Targeted Lung Health Check team in other administrative duties, as agreed with the Targeted Lung Health Check Responsible Assessor. The post holder will communicate with patients, colleagues, and external providers through a variety of different mediums to provide exceptional customer service to service users and colleagues.

Communication and relationship skills
• Deal with sensitive situations, including considering how the impact of any changes being made through improvement projects may affect the current working arrangements and practices of staff and manage communication methods accordingly.
• To answer telephone and email queries from patients, colleagues, and external providers.
• To provide expertise and project co-ordination in relation to improving the efficiency and effectiveness of the TLHC data capture and quality.
• To work as part of a team in partnership with Milton Keynes University Hospital Cancer Services, the Cancer Alliance, BMLK CCG, Respiratory Clinical Teams and any group or individual who use the TLHC service to support he delivery of high-quality data
Analytical skills
• To provide expertise and project co-ordination in relation to improving the efficiency and effectiveness of the TLHC data capture and quality
• Take responsibility for leading on specified work streams that support the improvement of the TLHC data
• To evaluate the working systems and existing processes within TLHC programme and redesign processes requiring improvement whilst also addressing any training and development requirements.
Planning and organisation skills
• To provide high quality day to day administrative support to the TLHC team, managing own workload, and using judgement and experience to help prioritise work.
• To attend MDT meetings as required and take relevant meeting notes
• The ability to work without direct supervision and on own initiative, managing the day to day office functions and liaising with departments both internal and external to the TLHC services as required.
