About the Service:
PALMS (positive behaviour,autism, learning disability and mental health service) works acrossHertfordshire providing a specialist multidisciplinary approach to children andyoung people aged 0-18 who have a global learning disability and/or autisticspectrum disorder and their families. PALMS clinicians comprise a virtualteam delivering a consistent service in a variety of community settings.
PALMS is a multi-disciplinaryteam who provide individually tailored support to help manage a number ofconcerns experienced at home. The service is available for children and youngpeople up to the age of 18.
Main duties of the job
The Responsibilities
If you are successful in yourapplication to this post, you will be expected to manage and deliver on theresponsibilities outlined below as well as those not included here, calltoday to discuss further:
To provide a comprehensive screening and triageservice to CYP and their referrers resulting in safe and appropriate clinicaldecisions and appropriate intervention.
Timely and effective triaging of all referralsadhering to the service specification, pathways and protocols to supportclinical decisions.
To be the first clinical point of contact forreferrers and the families who access, or enquire about accessing, PALMSfollowing agreed protocols and guidelines and utilising effective communicationskills. The calls can include assessing risk and the priority of the referral.
Ensure that sufficient, accurate and appropriateinformation is gathered in order to direct the referral to the most appropriatepathway.
To be a point of contact for team members todiscuss clinical queries that cannot wait until supervision or peer reflection.
To undertake priority assessments, related todifficulties within the PALMS remit, to determine the needs of the CYP,including whether it is safe for them to return to their home.

3. Significant experience of risk assessment within a mental health or learning disability setting.
4. Comprehensive assessment skills
5. Significant experience of working clinically in a mental health and/or learning disability setting assessing, containing, and managing complex cases in a multi-disciplinary setting.
6. Experience of working with people who have a learning disability and/or autistic spectrum disorder.
