* To work across the local community to provide a specialist service to patients in pulmonary rehabilitation.

Job responsibilities
* To work across the local community to provide a specialist service to patients in pulmonary rehabilitation.
* The post-holder will be a member of CSH Respiratory Care Team providing a high-quality pulmonary rehabilitation service.
* To be the lead Pulmonary Rehabilitation team member for a designated pulmonary rehabilitation group working in conjunction with the Highly Specialist Therapists and Nurses for the whole of the Pulmonary Rehabilitation Service.
* To independently perform advanced assessment of outpatients with diverse presentations and complex physical and psychological conditions to provide a diagnosis and deliver an individualised and specialised treatment programme of care.
* To manage own patient caseload and be responsible for a defined area of service, working without direct supervision.
* Supervision takes the form of regular formal training and competency assessment, appraisals, clinical reasoning sessions, and peer review.
* Access to advice and support from a senior physiotherapist is always available.
* To use evidence-based principles/current best practice to assess, plan, implement, and evaluate interventions.
* To undertake all aspects of clinical duties as an autonomous practitioner with freedom to act on own interpretation of data.
Person Specification
Qualifications
* Degree in Physiotherapy or equivalent.
Experience
* Experience of working with patients with cardiac & pulmonary conditions.
Qualification Evidence
* In addition to experience, all Qualified roles will be required to provide evidence of registration with the relevant Registration Body if applicable.
