Person Specification
Qualifications
* Proven evidence of an acceptable level of education/qualification (e.g. RMN/RGN, CSE, GCSE, O Level)
* A relevant vocational qualification or proven 'craft' skills to a minimum VQ Level 3 or equivalent
* Relevant ward-based experience in discharge planning IT skills - experience of Microsoft Excel, Word
Experience
* Experience working with patients and carer groups to develop innovative practice.
* Experience of managing clinical risks for patients with complex presentations.
* Lived experience of mental health issues/caring for older persons with mental health difficulties/dementia complex mental health needs/dementia.
Knowledge
* Knowledge of safeguarding both children and adults and patient safety.
* Awareness of Trust policies and service specifications.
* Understanding of key performance indicators and how they relate to team.
* Awareness of regional and national policies and procedures in relation to service area.
* NICE guidance and governance procedures in relation to patient safety.
Skills
* Demonstrate excellent verbal and written communication skills with staff, patients, families, carers, and professionals.
* Able to work with older people who have complex mental and physical health needs including dementia.
* Able to establish links with other services and professionals.
* To remain calm under pressure, to contain others' emotional distress and anxiety.
* Good communication skills both written and verbal.
* Proven team player with a flexible approach.
* Able to develop and maintain professional boundaries.
