Looking for a new challenge with more flexible working hours that fit your work-life balance? Our Community Nursing team in Wokingham, Berkshire is recruiting for District Nurses/Community Nursing Sisters/Charge Nurses.
In this role, you will provide support and leadership to a great team that takes pride in their commitment to the well-being of the community. Ideally, the candidate will have clinical experience working with adults in a community setting. In return, we offer an excellent package including opportunities for further training and development.
Your working hours will be Monday to Friday, with approximately one weekend every 4-5 weeks and bank holidays, along with some late shifts on a rotation basis. We welcome applicants who would like to work part-time or on a job share. A full UK driving licence and the use of a car for work purposes are essential for this role.

Main duties of the job
* Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
* Maintain detailed, accurate, and confidential patient records, adhering to Trust policies.
* Lead an effective supportive team, providing clinical advice and support to staff and patients.
* Manage a defined caseload of patients, with continuing responsibility for the assessment of care needs, and the development, implementation, and evaluation of care programs.
* Accountable for the standards of care and for determining the activity of your team, acting as a role model for staff and sharing learning.

Person specification
Education/Qualifications/Training
* Registered Nurse – Adult.
* Active on the Mentor Register.
* Evidence of Leadership & Management development.
* Recognised coaching qualification or willingness to undertake.
* Non-Medical Prescriber/Nurse Prescriber Qualification.
Previous Experience
* Experience in mentoring and assessing both pre and post-registration nurses in a community setting.
* Evidence of clinical leadership within a community nursing environment.
* At least 2 years Community Nursing experience at Band 6.
* Experience of leadership within a team.
Knowledge, Skills & Abilities
* Understanding of the role of Community Nursing.
* Able to work autonomously.
* High levels of effective communication, influencing, and interpersonal skills.
* Good organisational skills.
* Ability to motivate self and teams to positively manage change.
* Ability to influence and problem solve.
