We are seeking a highly skilled and motivated Clinical Nurse Specialist to join our dynamic team. As a Clinical Nurse Specialist in Thrombosis/Anticoagulation you'll play a pivotal role in providing expert care and support to patients dealing with clotting disorders. Our current service provides patient care across LUFT and within the Community setting, which includes Liverpool and Sefton areas.
This position offers an exciting opportunity to lead and influence patient care, collaborate with the interdisciplinary teams and contribute to advancements in Thrombosis/Anticoagulation management. The role will be working as part of the hospital and community team.
Assess, diagnose and manage patients with thrombotic conditions.
Develop individualized treatment plans in collaboration with healthcare teams.
Provide expert guidance, education and support to patients and families.
Monitor patient progress, response to treatment and manage any complications.
Lead and participate in educational programs for healthcare professionals and patients.
Conduct research, contribute to quality improvement initiatives and stay updated with advancements in thrombosis care.
Advocate for best practices in thrombosis prevention and management
