An exciting opportunity has arisen for a Band 5 Registered Nurse to join our forward-thinking West Suffolk Community Cardiac Rehabilitation Team, to contribute in the delivery of a high standard of holistic health care to patients within clinics, leisure centres and their own homes, by providing Cardiac Rehabilitation as part of the multi-disciplinary team.
This role can be offered as a developmental role and training can be provided for the right candidate who is keen to pursue an exciting and rewarding career in this evolving speciality.
Cardiac rehabilitation service provides person-centred care that enables individuals to increase independence, improve health outcomes and reduce unnecessary admissions to hospital or long-term care. This is achieved through timely interventions, rehabilitation, and management of long-term conditions.
We are looking for a passionate and motivated Nurse who is keen to develop evidence-based knowledge to provide specialist clinical care in both cardiovascular disease and other cardiac conditions. The successful candidate will need assessment skills to initiate clinical treatments, provide education and appropriately refer onward to our health and social care partners along with some voluntary sector services.
Main duties of the job
Key Areas of Responsibility:
1. Applicants will need the ability to travel frequently to rural locations so a full UK driving licence is essential.
2. To work within the community, liaising with the heart failure team, local hospitals, tertiary centres, local healthcare teams, GPs and practice nurses managing cardiac rehabilitation referrals at the Single Point of Access.
3. To provide support and advice for patients and their carers following their cardiac event.
4. To support the cardiac rehabilitation physical activity component of the programme.
5. To participate in CR group exercise sessions mirroring the patient.
6. To support the team in the delivery, development and audit of the service.
7. The post holder will have a caseload and will be supervised by senior members of the team.

Job responsibilities
MAIN RESPONSIBILITIES:
1. Triage, assess, plan, implement and evaluate all aspects of patient care, and develop care plans which may need to be delivered from a range of options.
2. Ensure informed consent is obtained and documented prior to initiating interventions.
3. Following a holistic assessment, devise an individualised, evidence-based plan of care for each patient, modifying it as required.
4. Assess patients holistic needs, communicating complex and sensitive information to patients and carers.
5. Identify and assess patients risk factors in relation to cardiovascular disease using recognised tools.
6. Formulate an individual plan of cardiac rehabilitation in collaboration with each client taking into account individuals ability and understanding in order to make lifestyle changes.
7. Use tactful and persuasive skills (such as motivational interviewing) to gain the patients co-operation in their care management plan.
8. Assess psychological function using recognised assessment scores e.g. HAD score or PHQ9/GAD7.
9. Assist senior staff with provision of education classes to support and advise patients with their rehabilitation following cardiac events.
10. Refer patients appropriately to dietician or smoking cessation services.
11. Participate in CR group exercise sessions mirroring the patient.
12. Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated.
13. Maintain skills with regards to latest technology.
14. Participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework.
15. Ensure own workload is planned and prioritised according to patient needs.
16. Ensure patient held records are completed for each contact, and that SystmOne inputting is completed on a daily basis.
17. Could be exposed to bodily fluids, infected material, blood products, therefore must utilise universal precautions and adhere to infection control policies.

Education and qualifications
* Current NMC Registration
* Documented evidence of Continuing Professional Development
* Commitment to undertake BACPR and Heart Manual training - this can be arranged on commencement of post.
* Basic ECG course
* Desire to complete Consultation and Assessment Course
Experience and knowledge
* Minimum of 1-year post qualification experience - to note this can be a developmental role with training provided.
* Knowledge and understanding of professional and ethical issues.
* Demonstrate a knowledge of Health and Safety legislation for self and others.
* To demonstrate a knowledge of Equality and Diversity.
* Experience of working within a community health setting.
* Sound clinical knowledge of current developments relating to NICE, ACPICR and BACPR guidance.
* Knowledge of research and evidence-based practice relevant to the role.
* Membership of a special interest group or organisation relevant to the role.
Skills and abilities

* Frequent Travel Essential - You will need to travel, so you must either hold a full, current driving licence and have access to personal transport or meet the mobility requirements of the role through other reasonable and suitable means.
