Job summary Blaenau-Gwent are seeking an exceptional Assistant Practitioner to join our dynamic Integrated Community Resource Team. As an Assistant Practitioner, you will have the chance to make a real difference in the lives of adults in need of support to remain in their own homes. You will be an integral part of the wider multidisciplinary service comprising of Social Workers, Physiotherapists, Rapid Response Team, Community Ward's, Reablement and Home Care. Main duties of the job Community Resource Team's (CRT), function is to work with people to enable them to be supported in their own home wherever possible, this includes providing a range of preventative interventions in conjunction with our Social Care partners. The Team is made up of a range of professionals from Health & Social Care. You will provide support in delivering prescribed clinical interventions and support using good problem solving, time management and assessment skills and supervisory skillsin a variety of community settings. In addition, you will also provide initial practical hands on support to people in their own homes working alongside the multi-professional team, undertaking delegated assessment and caseload managementas well assupervisory duties.We aim to deliver high quality health and social care services for adults in Blaenau Gwent and the role is an integral part of the CRT.If you are an experienced, caring supportive individual who enjoys working in a multi-disciplinary environment, we look forward to hearing from you.
