In your role as Family Practitioner, you will provide individual 121 support for parents and young people (up to age 18) as well as facilitate our group wellbeing programmes. During your work with families, you will explore their wellbeing goals and provide advice and guidance on whole family wellbeing strategies to promote improvements in emotional health and wellbeing.
Through listening to what really matters to parents, children, and young people, you will support the family to work towards the goals they set and link in with other services that can provide any additional help and support needed. You will be working towards the project targets and outcomes, supported by your line-manager and will be responsible for recording notes and evidence that reflects the work you do. You will work with the Local Authority’s Families First teams and Single Point of Access panel representatives to ensure that families get the right support at the right time.
You will also form part of a wider children and young people’s team, who work together across Platfform to contribute to ongoing service improvements and make the most of supervision, reflective practice, personal development and new ideas.
We work with people experiencing challenges with their mental health, and with communities who want to create a greater sense of wellbeing in the places that they live. Through our projects, we work with thousands of people each year.
We believe that long-lasting wellbeing comes about by understanding how lives can be shaped by traumatic experiences, identifying people’s strengths and focussing on healing. We know we can’t ‘fix’ people, but we can walk alongside people and help where we can on their journey.
