Are you looking for an exciting opportunity? We are inviting applications for an experienced band 3 Health Care Support Worker to join the Calderdale Learning Disabilities Nursing Team. The Calderdale Learning Disability Community Health Team is a multi-disciplinary team that provides specialist health support to both individuals, and their wider support networks. The administrative base is in the vibrant town of Hebden Bridge.
The team works agilely and covers the Calderdale locality. The work is varied and will give the right candidate the chance to develop a range of skills working alongside team colleagues in IST, occupational therapy, speech & language therapy, physiotherapy, psychology, psychiatry and admin.
An interest and/or experience in working creatively with people with a learning disability and/or Autism Spectrum would be an advantage.

Main duties of the job
We are seeking to recruit someone who matches our values and who can support us to continue to provide high standards of care and support. The post is a key role within the LD Community nursing team and the wider Calderdale LD health team.
This is an opportunity to work in a community team supporting the nurses and wider team to deliver evidence based clinical nursing support by supporting the assessment and information gathering and implementation of care plans.
Work collaboratively with service users and their carers as well as other professionals.
Job responsibilities
The post holder will:
* Under the supervision/direction of registered health care professionals/associate practitioner, within a delegated framework for agreed tasks, seek support as and when necessary, in the delivery of psychological and physical care to a defined group of people.
* As part of a healthcare team, provide the highest standards of safe and compassionate planned care to people that maximises their recovery and rehabilitation potential.
* Develop and maintain meaningful therapeutic relationships with people. Adopt a polite, positive and empathetic approach, communicating effectively and informatively.
* Demonstrate an understanding of the care and treatment programmes of a designated group of service users to enable you to instruct, advise & guide service users, family and carers.
* Be required to concentrate for periods of time, depending upon the care intervention/care plan, and respond to issues of concern appropriately.
* Exercise a degree of independence, initiative and judgement and consider different approaches when delivering tasks.
* Ensure the care experience is positive and service user safety is maintained. Treat everyone with respect, dignity, courtesy and in accordance with the Trust's Values.

Qualifications
* GCSE A-D or grade 3-9 in Maths and English or equivalent.
* Apprenticeship level 3 or equivalent.
* Care Certificate or required to achieve within 12 weeks of commencement.
* Willingness to undertake further study towards level 4 qualification.

* A Current driving licence and access to a car during the working day is essential.
Training
* Must be willing/able to undertake all appropriate Trust mandatory training requirements, and subsequent refresher training relevant to the area.
