We are looking for an Adult trained nurse who is seeking to develop new skills and is looking for a new challenge working alongside our care navigator and clinical operational lead to support our online consultations and our new neighbourhood team.
Main duties of the job
This role will primarily be working to cover the electronic e-hub consultations online for the Rural Rother Practices. You will advise/prescribe using an electronic consultations system with EMIS to patients within a remit of minor injuries and illness; full training will be given. We use a hybrid model of some office working/work from home, and you will need to be able to drive.
You will also support the development of our new neighbourhood team for the PCN. As a PCN, we are required to carry out holistic assessments to cover all health, social, and wellbeing aspects of the patients in care homes to develop personalised care and support plans. You will be required to identify the complex care needs of frail patients and support the care home staff using risk stratification tools such as the Rockwood score and electronic frailty index.

Rural Rother PCN is based in an office in Battle, East Sussex. We have an excellent team of clinical pharmacists, physios, paramedic practitioners, social prescribers, and care navigators that all provide a service to our patients in the rural Rother area. Our surgery teams work closely with each other, enjoying the ability to share expertise and resources to develop new services. Job responsibilities
You will have excellent IT skills and experience of EMIS or a similar clinical system. You will support our established e-hub with online consultations. You will be able to assess and manage patients presenting with a range of acute and chronic medical conditions such as acne, rashes, eczema, contraception, HRT, conjunctivitis, sore throats, urine infections, pain management, colds, and respiratory issues. Full training and clinical support will be provided during induction, and clinical mentorship will be offered ongoing. We also have GP supervision daily to discuss any consultations that you may need support with. You will:
* Recognise, assess, and refer patients presenting with mental health needs.
* Support patients in the use of prescribed medicines or over-the-counter medicines (within a scope of practice, reviewing annually as required).
* Maintain accurate clinical records in conjunction with current legislation.
* Ensure read codes are used effectively.
You will work as an autonomous practitioner evaluating patients with complex care needs by using physical assessment skills and independent nurse prescribing (V300). Assess, create, or update personalised care plans for vulnerable patients living in the local primary care network areas of Rye, Camber, Winchelsea, Sedlescombe, Westfield, Northiam, Broad Oak, Robertsbridge, and Burwash. Triage and manage acute and non-complex medical needs of patients who are housebound, palliative care, and vulnerable, within your scope of practice and knowledge.
Where other clinical support is needed, refer to GP or seek appropriate support from other community-based services. In conjunction with our care coordinators, work with other local service providers such as GP surgeries, community services, secondary care, and the voluntary sector to ensure that there is good communication between the multi-disciplinary services. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice or support for patients to access care for these issues. Implement and evaluate individual specialised treatment plans for chronic disease patients. Identify, manage, and support patients at risk of developing long-term conditions with the aim of preventing and managing the adverse effects these diseases may have on the patients' health. Provide advanced, specialist nursing care to patients as required in accordance with evidence-based guidelines, NICE, and the NSF.
Support the undertaking of simple diagnostic procedures such as undertaking ECGs and blood pressure monitoring, monitoring for sepsis, collecting pathological specimens, and requesting pathology services as necessary. Process and interpret pathology and other test results as required. Maintain accurate clinical records in conjunction with NHS guidelines for good practice. You will be at the heart of our primary care network program to support vulnerable patients, proactively working with practices, other voluntary agencies, and our care coordinators to identify and care for vulnerable patients in care homes and the local community. This includes those who have complex medical issues, palliative care patients, those with learning disabilities, autism, serious mental health illnesses, and those who struggle to access medical services directly from a GP practice and would benefit from support around managing their ongoing care.
You will visit patients in their own homes and in care homes, review and assess their health and wellbeing needs, design, and agree with the patients a personalised care plan. Then work with patients, their families, carers, and the wider multi-disciplinary team within community and secondary care to ensure that they can live the best possible life within the boundaries of their medical limitations. Our aim is for you to work alongside key community services already in place to provide coordinated personalised care for those in care homes and housebound patients to prevent unnecessary hospital admissions.

Qualifications
* BSc Nurse Practitioner/Community Specialist Practitioner and have studied at level 7. Experience of working in primary care/urgent care/A&E. Experience of working as a GP practice or relevant similar experience. Experience of carrying out thorough medical assessments of patients and creating treatment plans for their needs.
* Experience of prescribing and undertaking medication reviews or similar relevant experience.
* Qualified Independent Nurse Prescriber V300 essential for this role.
* Qualified Triage assessment practitioner or other relevant experience. Minor Illness Qualification or willingness to undertake training. Mentoring and supervision training undertaken. Experience of working with patients with long-term conditions/frailty.
