Registered Nurse - Community Resource Team

An opportunity has arisen for an enthusiastic Registered Nurse to join our Community Resources team in Wrexham. The post is to support improved service provision, to reduce unnecessary patient hospital admissions and support early discharge home from Hospital.
The hours of work are variable and will include weekend working. The geographical area of cover is across Wrexham. Full induction into this post will be supported, with opportunities for further career development.
Main Duties of the Job
The role of the Registered Nurse in this setting is to work across both acute and community services, to support the early discharge and admission avoidance of patients who can be supported at home with a range of community services, focusing on reablement and patient stability.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

* NMC Registered
* Evidence of post-registration study & training e.g., POVA

* Experience of providing holistic nursing care
* Clear understanding of clinical governance framework

* Ability to document details clearly and accurately
* Clear understanding of consent process

Disclosure and Barring Service Check
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
Salary: £30,420 to £37,030 per annum pro rata.
