We are looking to appoint a Care Navigator to join our South Wirral Community Team. You will be responsible for supporting the delivery of the personalisation and inclusion pathways for Adults and Carers ensuring compliance with the legal framework of the Care Act 2014.
You will ensure strength and asset-based practice, personalisation and social inclusion is embedded and applied effectively throughout. The role will involve assessing and implementing a range of social care services and providing guidance/advice and information for adults, older people and carers. You will work autonomously and independently, assessing, formulating, implementing, and agreeing care and support plans with adults and carers.
You will be responsible for supporting the delivery of health and social care functions for adults/carers for the people of Wirral. You will work in partnership with other agencies to support adults/carers in accessing services/support in the independent and non-statutory sector. About the Candidate
We are looking for someone who can be part of an integrated and supportive team, planning, monitoring and evaluating work with the MDT care team. You’ll need to have experience working in Hospital Discharge, supporting adults/carers/families and helping to promote self-determination, choice and autonomy. You will have knowledge and understanding of the Care Act (2014), Equality Act (2010), Mental Capacity Act (2005) and the Human Rights Act (1998).
Above all, you will be looking to improve the outcomes for the residents of Wirral and enhance the way we engage with our residents to ensure they are at the heart of everything we do.
