An exciting opportunity has arisen for Trainee Advanced Clinical Practitioners (t-ACPs) to be part of a new system wide model of care for our older population living with frailty. To join our Community Hospitals and Frailty team, to inreach to work within the Emergency Department at the Cumberland Infirmary Carlisle and West Cumberland Hospital Whitehaven. The post holder will cover a generalist population covering the community hospitals inpatient and new admission work. They will work with the Frailty team to perform assessments over a variety of areas (including the Emergency Department, SDEC, AMU and back of house wards). They will work towards completion of frailty competencies and curriculum.
Main duties of the job
The post holder will take part in delivering an acute frailty service which identifies and recognises older patients living with frailty and presenting with frailty syndromes. They will work collaboratively to perform a comprehensive geriatric assessment (CGA). The focus, where appropriate, is to return patients to their usual place of residence, or onward destination more suitable on the same day. This is through collaboration with our community partners, such as community beds, our integrated care communities, reablement services and virtual wards.
Within Community, there are 6 community hospital inpatient units. As part of this post you are expected, under supervision, to work with the medical staff undertaking assessments of patients in the units. The Community Hospital wards are a vital part of the healthcare system in North Cumbria. They are generalist wards and care for patients with a wide range of problems focusing on rehabilitation, complex discharge planning and end of life care. Many of the patients are frail and have multiple long-term conditions so need the expert holistic care. Each ward has a multidisciplinary team and you will be an integrated member of the team as a clinical decision maker, proactively reviewing patients, managing ongoing and acute medical problems as necessary, guiding and advising the team, communicating with families and contributing to the safe, effective care of patients on the ward.

Person Specification
Qualification
* Registered healthcare practitioner with substantial post education experience in either frailty, acute services or a generalist speciality.
* Educated to degree level.
* Training and mentorship qualification or equivalent
* Non medical Prescribing V300 or willingness to work towards it.
* Relevant validated clinical courses.
* Clinical Educators course
* Advanced life Support
* Leadership qualification or equivalent
* Additional clinical qualifications in frailty / older persons care
Knowledge
* Good knowledge of current best practice in relevant clinical area
* Knowledge and understanding of Clinical Governance and its implications for practice
* Awareness of Risk Management
* Knowledge of audit, evaluation and use of outcome measures
* Professional knowledge acquired through degree/diploma supplemented by significant post-registration experience and specialist knowledge of the clinical area applied for
* Experience of mentoring and developing staff

* Evidence of management skills gained through theoretical knowledge or experience
Experience
* Demonstrates knowledge of comprehensive geriatric assessment and also undifferentiated diagnosis as well as long-term conditions with the delivery of expert holistic clinical care to patients

* Experience in emergency/acute or pre-hospital medicine
* Experience managing critically ill or unwell patients
