Job summary Job Summary The post holder will be part of an integrated service inclusive of health and social care staff, working collaboratively with both primary and secondary care professionals. The post holder shall act as an expert resource for the multidisciplinary team and work collaboratively with Care Home staff and management for all patients that are placed onto Pathway One, Two or Three and awaiting further health and social care assessments. The post holder will review and assess patients that are Walsall residents or have a registered Walsall G.P on ICS Pathways One, Two or Three with an identified Mental Health condition or need. The post holder should be working towards their V300 prescribing or have the desire to complete the training. Additionally, the post holder needs to maintain their own professional development in relation to mental health nursing practice.
