There are a number of posts we are recruiting to within the new structure to include: Service Manager – Early Support Service Manager – Hospital and Reablement Service Manager – Disabilities Service Manager – Ageing Well Service Manager – Safeguarding As service manager for Hospital and Reablement you will lead the hospital social work and reablement teams to provide high-quality care, including assessments, interventions, and discharge planning. Ensuring services meet local authority guidelines and integrate effectively with the wider healthcare system. You will play a key role in the delivery of integrated services to adults across the services you lead, ensuring appropriate focus on those most in need/at risk within the context of the wider service with emphasis on preventing, reducing and delaying the need for long term care and support. You will also oversee one of the inhouse regulated services Bradley Resources Centre, and report into the Head of Service for Disabilities and Mental Health.
