This post is involves working families of school age children who have additional needs The post holder must hold a current UK driving licence Implement planned programmes of care following assessment by SCPHN/Staff Nurse, Promote the needs of children with SEND, young people with SEND and their families in the development of community child health services working in partnership with parents and carers and partner agencies in a variety of settings to help them create an environment where children and young people can develop and learn. Work in partnership with parents and carers to build on their skills so that childrens and young peoples wellbeing, resilience and life opportunities are promoted. Establish and maintain positive interpersonal relationships with other team members. Positive relationships are characterised by open communication, trust and respect. Promote effective team working, sharing knowledge and information, encouraging a supportive environment to promote good working practices and outcomes. Provide a standardised, consistent approach (applying the evidence base) to the identified, individual needs of the child, young person and family. Contribute to the development, delivery and reporting of health promoting activities within a variety of settings. Deliver evidence based early intervention programmes to support children, young people and families e.g. Speech and language development, prevention of accidents in the home, promotion of oral health, promotion of healthy weight and physical activity. Signpost and provide information for parents and young people, communicating information in an understandable form to parents, children and young people. Work flexibly by undertaking any other duties (within scope of practice) compatible with the post as directed by the management team SCPHN/Staff Nurse in relation to service needs
