Job Summary
An opportunity has arisen for an enthusiastic and motivated individual to join our well-established Heart Failure service.
The Heart Failure Clinical Nurse Specialist will work within a multi-disciplinary team, providing clinical assessment and management, health promotional support, and teaching patients and staff about heart failure. The role also involves participation in research projects and supporting the development of the heart failure service.
This is a fixed-term post for 12 months due to funding.
Main Duties of the Job
1. Clinical assessment and management of patients with heart failure.
2. Support patient/carer and staff education around heart failure.
3. Support research projects.
4. Support service development.

Job Responsibilities
1. Provide clinical and health promotional management to patients presenting with heart failure.
2. Utilize Non-medical Independent prescribing to support patient care.
3. Deliver education to patients and carers regarding heart failure management.
4. Provide teaching to a multi-professional audience on heart failure presentation and management.
5. Complete Heart Failure competencies and maintain a live portfolio to support enhanced level practice.
6. Engage in key research projects and utilize research to support clinical management decisions.
7. Support the development of the heart failure service.
Person Specification
Clinical
Essential:
* Experience in cardiology/heart failure care and management.
* Completion of clinical assessment and diagnostic level 7 module.
* In receipt of or working toward non-medical independent prescribing.
Desirable:
* Completion of Level 7 Heart Failure Module.
* Completion of heart failure competencies.
Research
Essential:
* Involvement in research projects.
* Evidence to support evidence-based clinical practice.
Desirable:
* Lead on research-based projects.
Education
Essential:
* Evidence of delivering education to patients and carers.
* Evidence of delivering education to other professionals.
* Holds a live portfolio reflecting Enhanced Level practice as outlined by HEIW.
Desirable:
* Formal teaching qualification.
Leadership
Essential:
* Evidence of supporting service change.
Desirable:
* Evidence of leading change.
* Formal leadership qualification.
