We are looking for someone to join our large practice for 20 hours per week. We need someone to assist with our recalling patients' procedures to ensure they are booked in to see the appropriate clinician and seen in a timely manner.
Main duties of the job
1. Help generate call/recall reports highlighting patients due for appointments in a clear and organised way.
2. Contact patients to book initial appointments and remind them for booked appointments for their chronic disease management.
3. Support with the follow-up of appointments and patient DNA.
4. Help coordinate and recall patients for their annual reviews (e.g., Diabetes, CVD, Integrated Care, etc.).
5. Support with the systematic recording of appointments and attendance history on the practice shared drive.
6. Attend meetings and share updates with the wider team.

Job responsibilities
1. Your main responsibility will be to coordinate and navigate the care of patients with long-term conditions.
2. You will be in charge of booking them for all their check-ups and ensuring that these are followed up if needed.
3. Create and run searches on EMIS Web.
4. Share updates with other team members.
5. Invite patients for Diabetes, NHS Health check, CVD, CHD, and stroke check-ups.
6. Call patients on the integrated care list and complete templates.
7. Keep updated with any changes in the specifications on a yearly basis.
8. Apply the correct code and share information with the wider team.
9. Organise integrated care MDT.
10. Fill all HCA sessions each week.
11. Keep a tidy and organised recall system for all long-term conditions.
