Single Point of Contact – Clinical Nurse Specialist
An exciting opportunity has arisen in our Single Point of Contact (SPoC) team for a Clinical Nurse Specialist or Allied Health Care Professional with excellent organisational skills and clinical experience to provide person-centred care to individuals in various settings throughout the local community.
We are seeking experienced registered nurses or allied health professionals with excellent clinical decision-making skills and the ability to communicate tactfully and understandingly, with a wide range of people, including outside agencies, patients and carers.

Hours: 22.5 hours Monday – Friday (with some flexibility required to meet patient needs)
What is the role?
As a SPoC Clinical Nurse Specialist or equivalent Allied Health Care Professionals, you will be the first point of contact for our patients, families, and healthcare professionals, managing referrals, the advice line, introducing our services, providing advice and making correct onward referrals at the right time. You’ll be doing the initial holistic patient needs assessments, predominantly by phone or within an outpatient setting, providing expert advice, and ensuring our patients receive timely and holistic care, tailored to what matters most to them.
Working closely with a multidisciplinary team, you will collaborate to deliver exceptional care while also offering professional guidance and support to other healthcare providers. As a key part of the team, your expertise will ensure that patients and their loved ones receive the best care possible during difficult times.

About you:
* Registered General Nurse/Health or Social Care Professional
* 5 years post-registration experience
* Experienced in the field of palliative care and in assessing patient needs that may be complex and subject to change
* A good understanding of symptom control management within palliative care and confident to make safe recommendations
