A Vacancy at West Hertfordshire Teaching Hospitals NHS Trust.
The Virtual Hospital at West Herts comprises of collaboration between West Herts and our community partners at CLCH. The purpose of the virtual hospital is to treat acutely unwell patients in the comfort of their own place of residence, by remotely monitoring them and providing home visits where necessary. Our vision is to support the patient’s journey in a way that improves overall patient experience whilst also improving bed flow in the acute wards by preventing admission and supporting early discharge.
The role of the junior clinical fellow in the Virtual Hospital is to support the daily management, review and flow of patient on the Virtual Ward.
Currently, the VH offers the following pathways – Heart Function (HF), Asthma, Bronchiectasis and COPD (ABC), Acute Respiratory Infections (soon to be General Medicine instead), Frailty and lastly a small Post-Surgical Colorectal pathway.
Not only do we provide specialist care for our patients, but we work to provide strong wrap around care to prevent readmissions.

General ward work and on-going care of in-patients- including remote ward rounds for inpatients.
Review of patients referred to the virtual ward and face to face reviews of inpatients as required, to screen for suitability for VH onboarding.
Maintenance of ward lists.
Attendance to consultant ward rounds.
Attendance at MDTs.

The duties of the post will be carried out in conjunction with the Consultant by means of daily ward rounds and follow-up care. The Post holder is expected to undertake appropriate investigations and therapeutic procedures (under supervision).
They will also maintain adequate medical records of the patients’ stay in hospital and dictate discharge summaries and enquiries so that they are promptly sent out to the General Practitioner. Patients are admitted from hospital wards or from the community via the SDEC (same day emergency care), Acute Admissions Unit or via specialist clinics.
