Campingland Surgery has a fantastic opportunity for an experienced nurse practitioner to join our team on a part-time basis, for 22.5 hours per week on a Monday, Wednesday and Friday. This will include a mix of care home and in surgery clinics.
Main duties of the job
* Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
* Providing and evaluating a range of health interventions to ensure that patients' health and social care needs are met.
* General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
* Managing and treating long-term conditions, in line with national and local policies and the practice needs.
* Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
* Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
* Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
* Meeting the needs of patients presenting for opportunistic wound care.
* Hypertension review and management.
* Running asthma, COPD and diabetes clinics and reviews.
* New patient and health checks.

Our surgery is situated in the heart of Swaffham town centre with 5 partners, and a practice population of approximately 7500 patients. We are a busy training practice and have 2 care homes within a short distance of the surgery that are also part of our patient population.

Person Specification
Qualifications
* Current NMC registration with an active PIN.
* Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
* Competence to undertake asthma, COPD and diabetic clinics and reviews, holding relevant certificates.
* Qualified Prescriber.
* Completion of Enhanced Clinical assessment module.
Experience
* Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care, and recording spirometry.
* Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work.
* Knowledge, and experience of its application, in supporting long-term conditions.
* Knowledge and experience of working within the Care home setting including ward rounds with Care home staff.
* Insight and understanding of current issues in nursing and the wider NHS.
