A rare opportunity has arisen for a band 5 registered nurse to work within the Urgent Community Response (UCR) team as part of the Intermediate care service.
We are looking for a dedicated and passionate band 5 nurse to join the team. The ideal candidate will assess patients within the Hull and East Riding geographical area and also service users based in a bedded unit. The ideal candidate should have some community experience; however, full training and support will be provided.
The post holder will work autonomously and also as part of a team to provide safe, effective, efficient care for patients, relatives, and carers. They will be responsible for the assessment, planning, implementation, and evaluation of programmes of care under indirect supervision from senior staff.
Main duties of the job
* To support senior and to supervise junior staff in the provision of competent, sensitive, and safe nursing care within the Community Ward/Community Short Stay Unit
* Take responsibility for running of the department during a shift in the absence of more senior staff
* Take responsibility for assessment, planning, implementation, and evaluation of nursing care for a group of patients

Qualifications
* Be willing to undertake further training and development
* Completion of post qualification preceptorship
Knowledge & Experience
* To have recently worked within a nursing capacity
* Demonstrate an understanding of the MDT
* Community Hospital experience
* Demonstrate an interest in palliative care
* Demonstrate an understanding of evidence-based practice
* Have an interest in audit and maintaining standards
