Job summary
We are looking for a practice based Clinical Pharmacist responsiblefor support to the practice with medication reviews, queries and prescribingsafety.
The new person will also be responsible forsupporting the partners in implementing effective medicine management withinthe practice. Supporting the pharmacyteam to identify areas for improvement and then initiate and manage the changes.
The successful candidate will also be responsible in leading the practice medicine monitoring reviews and supporting our pharmacy technician and prescribing clerk.
Main duties of the job
The post holder will work within their clinical competencies as part of amulti-disciplinary team to provide expertise in all aspects of medicinesoptimisation within the practice:
1. Lead and oversee the high risk medication monitoring with the practice admin and the in-house pharmacy team.
2. Provide a point of contact for the practice for all medicines-related queries for healthcare professionals as well as patients.
3. Patient facing clinics are not currently required but if there are areas of specialty that could provide benefits to the practice then the surgery would be open to suggestions.
4. Follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
5. Oversee the MHRA alerts/CAS alerts, to make sure these are audited, actioned and logged on a spreadsheet.

6. Reconciliation of medicines from outpatient and discharge letters including liaison with hospital, community and primary care colleagues to ensure correct medicines are continued following transfer of care.
7. To work with the pharmacy team and support GPs and other practice staff to deliver on Medicines Optimisation agenda (Prescribing Quality Scheme), QOF and locally commissioned enhanced services.
8. To encourage, support and implement adherence to local formulary guidelines and cost-effective prescribing.
9. Reviewing, issuing and converting acute medicine requests into repeat medicines, where appropriate.
10. Supporting Community Pharmacists in ensuring NOMAD or batch prescriptions are up to date including confirming any stopped or initiated medications with the patient and community pharmacy team.
11. Implementing and monitoring a practices adherence to a repeat prescription policy.
12. Supporting the Practice Pharmacy Technician (or prescription clerks) with the management of the repeat prescription processes and other medicines management related duties.
13. Contributing to repeat prescribing safety prompting patients for reviews, blood tests, monitoring etc.
14. Working with the multidisciplinary team to improve medicines management and safety within the practice including updating, writing and ensuring compliance with practice policies.
15. Dealing with patients medicine and prescription related queries over the phone, in clinic, via letter/email or the practice website (this may include advising and prescribing for minor ailments or clinical areas within competence, acute prescription requests, carrying out medication reviews, lead on medication monitoring, providing medication advice, solving technical prescription problems such as supply problems, NOMAD changes etc.)
16. Signposting patients to appropriate services and other healthcare professionals ( community pharmacists).
17. Working closely with GPs to provide medicine related updates and helping with problem solving.

48. Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
49. Holds or working towards an Independent prescribing course
50. Postgraduate Diploma in Clinical Pharmacy

52. Previous experience in hospital, general practice or CCG
53. Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice

56. Minimum of 2 years post qualifying experience
