About the role:
An opportunity has arisen for a Shared Lives Powys Scheme Officer Position in the Social Care Delivery Team. This position is a full time, permanent position. Shared Lives is a Registered Service under the Regulation and Inspection of Social Care (Wales) Act 2016 and Adult Placement Regulations (2019). The position is pay grade 7.
About you:
• Ability to work effectively on own initiative as well as part of a team
• Ability to communicate effectively both written/verbally & proficient in IT systems
• Ability to manage, and prioritise workload and work from a proactive stance
• Understanding of legislative and policy frameworks such as Regulation and Inspection of Social Care (Wales) Act 2016 and Adult Placement Regulations (2019)
What you will do:
• To recruit, assess, train, support and monitor Shared Lives Carers providing support and accommodation in their own home, on a long or short-term basis, to adults in need of support within Powys.
• Match Shared Lives Carers with potential service users and facilitate introductions between all parties and monitor arrangements to ensure the satisfaction of all parties.
• To work within council and service policy and procedures, and the Regulation and Inspection of Social Care (Wales) Act 2016, and Adult Placement Services Statutory Guidance.
• To assess potential Shared Lives Carers, produce assessment reports and present them to the approval panel.
• To contribute to the identification of service users’ outcomes, strengths and capacity. To produce an appropriate plan identifying how those outcomes will be achieved and what support is needed.
• To be responsible for a designated case load of Shared Lives Carers and arrangements
