Duties and Responsibilities: The post holder will be a member of the SWL Medicines Optimisation team and will contribute to the operational delivery of the teams objectives and deliverable, working with multidisciplinary providers and stakeholders, and PCN teams including PCN ARRS clinical pharmacists They will: Responsible for working collaboratively with PCNs to deliver the Place savings plans, quality and safety pieces of work as per the annual workplan ensuring safe, evidence-based cost effective prescribing whilst also identifying areas of improvement for allocated Place(s) and working with them to develop a plan on how to improve medicine optimisatiaon performance in key areas. Responsible for supporting GPs, PCN/GP pharmacists, nurses, community pharmacy staff within allocated PCN with agreeing annual priorities/areas of focus for the upcoming year, induction for new medicines optimisation related employees, clinical queries where appropriate, education and development, performance against local and national targets/indicators. Support SWL clinical network(s) as allocated and the lead for that area with guideline and pathway development. Support for implementation of community pharmacy service integration at Place and across SWL. Support Medicines Optimisation Governance, Safety, Quality, Care Homes, Value and Productivity as appropriate.
