An opportunity has arisen for an experienced midwife to join the Maternity Governance Team. The post holder will be part of a team of senior midwives that lead on implementing governance processes, across the service, in collaboration with the Quality and Safety Matron and Head of Midwifery. The post holder will participate and lead in reviewing incidents and adhering to statutory reporting processes for serious incidents that involve MNSI, PMRT or StEIS; liaising with families to ensure that their voices are heard in the investigation process and that the service adheres to applying the principles for Duty of Candour.
The post holder will be responsible for reviewing, monitoring, and evaluating incident reports in conjunction with the senior midwifery team to ensure that timely review, investigation, and escalation occurs. Ensuring that any lessons learned are fed back to the staff involved and the service, facilitating organisational learning.
An Enhanced DBS with barred lists for both Adults & Children is required for this post.
Main duties of the job
* Be responsible for managing incident reporting ensuring relevant reviews are undertaken, incidents closed with learning disseminated
* Provide professional and managerial support in relation to clinical risk.
* Where appropriate deputise for the Quality and Safety Matron
* Assist the Quality and Safety Maternity team in achieving and maintaining high CQC standards and achieving MBRRACE recommendations, the NPSA and or any other relevant agencies.
* Identify incidents that require reporting to the Healthcare Safety Investigation Branch (HSIB) and support the reporting and investigation process for these cases.
* Participate and support the Perinatal Mortality Review Process, notifying cases and contributing to case surveillance. Drafting and reviewing reports in partnership with the Quality and Safety Matron and the Obstetric Lead.
* Coordinate Avoiding Term Admissions Into Neonatal (ATAIN) reviews in partnership with the NNU Team and ensure that maternity learning is disseminated.
* Lead in the co-ordination of directorate risk activities including Serious Incidents and other risk reporting; including near miss reporting, Duty of Candour, and ensuring that learning is disseminated and effective to reduce re-occurrence of event.

Job responsibilities
KEY DUTIES

* Provide professional and managerial support in relation to clinical risk.

* Ensure that all investigations are completed in a timely fashion and recommended actions are completed accordingly.
* Co-ordinate and assist in proactive risk assessment processes.
* Co-ordinate and assist with maintaining the O&G risk register.
* Assist with maintaining the maternity dashboard.
* Update all staff on risk related issues through mandatory training and information dissemination in all formats.
* Write detailed reports as appropriate and enable other staff to generate fit for purpose incident/Governance reports.
* Visibly and proactively contribute daily to the Safety Huddle process by sharing lessons learned from incidents, and risk reviews in a timely manner.
* Provide monthly updates on clinical incidents to the O&G Clinical Governance and Quality Group by the production of the monthly Safety Dashboard report.
* Facilitate practice change in response to lessons learned from the Governance Framework and actively encourage involvement of appropriate personnel e.g, Midwifery Matrons/Managers, Specialist Midwives, Paediatric Consultants, Anaesthetists, Theatre Team, escalating to Head of Midwifery when appropriate.
* Co-ordinate and propose improvements regarding the incident reporting system in order to monitor trends in relation to adverse incidents.
* Ensure Lead Consultant for Governance; Head of Midwifery and Quality and Safety Matron receive lessons learned as soon as they are available.
* Provide the Head of Midwifery; Consultant Obstetric Lead for Governance and Quality and Safety Matron of any findings of incident reviews, which pertain to staff practice issues to enable them to direct appropriate action if required
* Attend and provide regular feedback and update as a member of the O&G Clinical Governance and Quality Group; to Labour Ward & Maternity Forum; Professional Forum; Senior Managers Meeting; Obstetric and Gynaecology Directorate Meeting and any other appropriate meetings.
* Support the Quality and Safety Matron, Head of Midwifery and Consultant Obstetric Lead for Governance by liaising with the legal department when appropriate in relation to incidents and potential or actual claims.
* Work closely with the Obstetric Clinical Lead for Governance, and the Senior Maternity Management Team to achieve the full implementation of the objectives outlined in the Directorates Governance Strategy.
The post holder will proactively contribute to the following objectives in the Directorates Governance Strategy:
* Ensure that Governance is embedded into the maternity culture.
* Ensure all staff within the maternity unit are aware of their role in enabling effective Governance.
* Ensure all staff within the maternity are competent to enable effective Governance.

* Empower all staff within the maternity unit to report risks and concerns.
* Ensure all risks are reported.
* Reduce and effectively manage risks within the maternity unit.
* Ensure all reported risks are analysed for emerging themes and the need for corrective action plans.
* Ensure action plans are developed, implemented and monitored for reported risks that require this level of intervention.
* Integrate incidents, complaints and legal claims through the production of formal reports.
* Comply with governance requirements of the Trust.

* Attend relevant training and development in order to maintain and develop skills and knowledge required for revalidating as a practising midwife.
* Undertake annual mandatory training updates and other relevant courses in line with trust and local policies.
* Present an appearance that promotes a positive profile of the midwifery service and the Trust, in line with the Trust uniform policy.

Person Specification
Qualifications
* Registered Midwife with current NMC Registration.
* Evidence of degree or evidence of degree level study or willingness to work towards.
* Evidence of post registration education.
Knowledge & Experience
* Works well under pressure and has demonstrable experience of meeting project deadlines.
* Knowledge and awareness of the principles of governance in Healthcare and Maternity.
* Evidence of experience and understanding of governance processes.
