Responsibilities :
* Patient and Family Education : They educate patients and their families about illnesses, treatment plans, and available resources
* Psychosocial Assessments : Conducting assessments to identify any mental or emotional distress and addressing these issues.
* Counseling : Providing counseling to patients and families in crisis or experiencing distress
* Discharge Planning : Developing and implementing individualized care plans to ensure safe and effective discharge from the hospital.
* Coordination of Care : Working with interdisciplinary teams, including doctors, nurses, and case managers, to coordinate patient care and ensure seamless transitions.
* Advocacy : Advocating for the best interests and wishes of the patients, ensuring their voices are heard in care decisions
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
