Our services operate 24/7 providing vital patient care from Ashton Primary Care Centre, Manchester Royal Infirmary, North Manchester General Hospital, Wythenshawe Hospital and Denton Clinical Hub. We are recruiting flexible, part time roles, up to 30 hours a week, across midweek evening and overnight shifts and weekend daytime, evening and overnight shifts. Care Co-ordinators are the first point of contact for health care professionals, patients and relatives of patients accessing NHS urgent and non-urgent care services, they are pivotal in the smooth running of our services and ensure patients are directed to the most appropriate healthcare service or pathway. Driving (optional role addition) Within the Care Co-ordinators service, Driving Care Co-ordinators support our clinical teams with a home visiting service, to bring healthcare professionals to the patient when its required. If desired, driving shifts are available as part of your care co-ordinator role. Drivers must hold full UK license, be over the age of 25 and no more than 3 license points to operate our vehicle. Main duties of the job The role of the care co-ordinator is fundamental to the smooth running of the out of hours and urgent care service.
