Primary Care Mental Health Liaison Practitioner - Incentive Applies
The Chichester Assessment & Treatment Service is a community mental health team that supports people living with a severe and enduring mental illness. The MHLP team is part of the Chichester Assessment and Treatment Service and works as a liaison between the ATS and GP Surgeries. The post holder will work within GP surgeries and will be the liaison and link between primary care and secondary care services.
We are looking for an experienced Band 6 Mental Health Practitioner (Mental Health Nurse, Occupational Therapist, and Social Worker) to work within GP Surgeries to provide 6-8 sessions of CBT informed or psychoeducational intervention to people with moderate mental health problems. The successful applicant will also be responsible for completing initial assessments and liaising with GPs, IAPT services, local agencies, and signposting to third sector services.
A band 5 - 6 Developmental post and Part-Time Applicants will be considered.
This post offers the Trust's £8,000 Incentive for band 5-7 Nurses & Practitioners as well as a relocation package up to a maximum of £8,000 (subject to terms and conditions).
Main duties of the job
Working as an MHLP includes:
* Working autonomously to a defined caseload of clients diagnosed with mild to moderate mental health problems, including offering specific interventions such as psycho-education, CBT informed sessions, and short-term recovery-focused interventions.
* Completing initial assessments and working closely with our referrals coordinators, GPs, and other agencies.
* Providing a pre-defined offer of 6-8 sessions of intervention.
* Co-facilitating groups if you have an interest in this area.

Job responsibilities
To work as part of the secondary care mental health service, acting as a link clinician to primary care teams. Clinical work will be carried out within primary care settings.
* Working with GPs and primary care teams, specifically colleagues in IAPT (Time to talk), providing support, advice, consultation, and shared care for adults presenting with a functional mental illness.
* Collaborating with GPs and other primary care colleagues to develop care plans for clients.
* Assessing individuals with complex presentations prior to referral to secondary care and directing them into appropriate specialist services.
* Maintaining a close working relationship with local secondary care teams, keeping them informed of developments within primary care.
* Building professional relationships within Primary Care to develop care pathways within localities.
* Working closely with secondary care colleagues to ensure appropriate responses to referrals.
* Keeping updated on local resources and services available, providing this information to practices and specialist teams as requested.
* Providing comprehensive information on the role of the Primary Care Mental Health Liaison Practitioner and local resources to GPs and primary care teams.
* Working to early intervention principles and providing assessments for service users with complex presentations.
* Organising and coordinating training to GPs and primary care teams on mental health topics as agreed with GPs and PBCs.
Person Specification
ESSENTIAL
* Registered Mental Health qualification or equivalent.
* Post Basic Courses / relevant Study Days.
* Evidence of CPD.
Experience
* Post-qualifying experience of clinical work with a range of mental health problems.
* Experience of partnership working within and across various statutory and non-statutory teams and agencies.
* Experience of managing a caseload and prioritising clinical work.
* Experience of providing clinical interventions to individuals with various mental health problems.
* Experience of working consultatively with professionals.
* Experience in conducting clinical assessments, including risk assessments.

£35,392 to £42,618 a year PA / pro rata for part-time.
