Derbyshire Dales Primary Care Network (DDPCN) are seeking a compassionate and reliable Hospital-to-Home Driver to provide safe, comfortable, and reliable transportation for discharged patients from Chesterfield Royal Hospital and Royal Derby Hospital to their homes in the following geographical areas:
1. Amber Valley
2. Erewash
3. Chesterfield
The driver will be responsible for transporting patients from the hospital to their homes and ensuring they are settled back in their own living spaces. This role requires a strong sense of empathy, excellent driving skills, and the ability to assist patients with basic needs during the transition process.
The operating hours of the service will be between 10:00 a.m. and 6:00 p.m. Monday to Friday between 1st May and 30th October. This will be increased to include Saturdays between 1st November and 30th April annually.
Main duties of the job
Safe Transportation: Transport discharged patients from the hospital to their homes using a company-provided vehicle, ensuring their comfort, safety, and well-being throughout the journey.
Patient Assistance: Help patients with basic mobility, including assisting them in and out of the vehicle, offering support when required, and ensuring they are safely settled at their home.
Coordination: Work closely with hospital staff and family members to ensure a smooth handover of patients and confirm all necessary care instructions are understood.
Home Settle-In: Assist patients with entering their homes, ensuring the environment is safe, clean, and conducive to their post-discharge care. Provide assistance with unpacking or arranging immediate necessities, such as medications or comfort items.
Documentation: Keep accurate records of each transfer, including patient information, delivery times, and any relevant observations that may need to be shared with family members or healthcare providers. This information will be captured electronically via a laptop or tablet on a secure network.
Communication: Maintain clear and professional communication with patients, family members, hospital staff, and the office team. Report any concerns or changes in patient condition.
Vehicle Maintenance: Ensure the vehicle is clean, fuelled, and well-maintained. Conduct regular checks to ensure the vehicle is in safe working condition.

This role will be conducted on a 'remote working' basis whilst maintaining daily contact and support with colleagues at the DDPCN office. You will be a valued member of the 'team' working principally from the Chesterfield or Derby Hospitals.
Job responsibilities
Valid Drivers Licence: Must have a clean, valid driver's licence and a reliable driving record.
Experience: Previous experience as a driver, preferably in a healthcare, public service or patient transport role, is an asset.
Knowledge of Basic Patient Care: While formal medical training is not required, experience or knowledge of assisting patients with mobility, post-hospital care, or other health-related tasks is preferred.
Communication Skills: Ability to communicate clearly and empathetically with patients, their families, and healthcare providers.
Physical Fitness: Must be able to assist patients with mobility, lift items as necessary, and help with basic physical tasks such as assisting with entry/exit from the vehicle and home.
The vehicle will be a WAV (Wheelchair Access Vehicle).
Background Check: Must pass an enhanced DBS check as part of the hiring process.

Reliable and Punctual: Consistently arrive on time for scheduled transfers and ensure that patients are transported promptly.

Detail-Oriented: Ensure that all aspects of the patient transfer, including communication and paperwork, are accurately completed.

* Full Driver's Licence.
* Background in health care or public service where driving and transportation of people was a part of the role.
* Understanding of Safeguarding principles.
* Flexible working pattern including Saturdays when needed.
* Be able to secure an enhanced DBS check/security clearance.
* Background in public services.
* Capable of lone working.
