Job summary Due to the exciting transformation of the Memory Assessment Service we are currently seeking highly skilled, motivated, passionate practitioners with a desire to provide high quality, timely care. Main duties of the job To provide thorough memory assessments for those referred into the service To gather a detailed collaborative history of presenting problem from the patient and their carer To gather relevant past medical history from system partners To be able to undertake physical health observations and awareness of escalation routes To formulate plan of treatment in collaboration with senior colleague in MDT. To assess risk and develop management plans, reviews and adjust in line with the service user's needs.
