Our client, a leading organisation in supported housing, is seeking a dedicated Support Worker to join their Complex Needs team. About the Role: As a Support Worker, you will play a crucial role in empowering individuals at risk of homelessness to rebuild their lives and work towards independent living. Working within a small, supportive team and guided by a Team Leader and Team Manager, you will provide high-quality, person-centred support to a small caseload of customers. Key Responsibilities: Develop, review, and implement personalised support plans and risk assessments. Adopt a holistic approach to support, assisting individuals in achieving personal goals. Collaborate with external agencies to address specific needs, including mental health, substance misuse, and financial support. Guide customers in exploring education, training, and employment opportunities. Help individuals maintain tenancies and prevent homelessness through proactive support. Manage risk effectively and take a solution-focused approach to challenges. About You: To be successful in this role, you will need: A strong understanding of support planning and risk management. Experience working with individuals with complex needs. Excellent communication skills and the ability to build meaningful relationships. Strong IT and record-keeping skills. Confidence in lone working as well as being a proactive team player. Adaptability, resilience, and a solution-focused mindset. A flexible approach to work and the ability to manage pressures effectively. This role requires an enhanced DBS check.
