Main Duties: An essential part of this role will be focussing on the holistic review of patients with Learning Disabilities aged 14 and over. The PCN has a well established and successful process for conducting the reviews from practices and patients' homes and we are looking to continue this process whilst being open to suggestions for improvements, whilst maintaining wider collaboration with other system partners to ensure we provide the best care and support. Another established workflow is to take part in Enhanced Health in Care Homes part of DES contract, which entails preparing and updating Personalised Care and Support Plans for care home residents. In addition, there may be a requirement to provide leadership and mentorship to trainee Nursing Associates within the PCN, and you will work with ourPCN PracticeNurses to support with the development, rollout and running of PCN wide projects and services.
