Are you a qualified nursing associate looking for an exciting new challenge and would like to be part of our new Acute Response Team? If so, we are searching for Band 4 Nursing Associates to join our new and evolving nursing team in CHFT Acute Response service.
This service will support the deteriorating patient on both sites and will be working with the new integrated team (formerly Critical Care Outreach and Hospital Out of Hours).
The post holder will work as an integral member of the Acute Response team, providing an exemplary service to deteriorating patients.
The post holder will utilise an appropriate assessment tool to determine allocation of tasks to skilled professionals, as well as utilise their own learned skills, including:
1. Triaging patients
2. Venepuncture and cannulation
3. Collection of blood cultures
4. Ultrasound guided cannulation
5. Venous blood gas sampling
6. Spirometry out of hours
Be involved in informal and formal education to other healthcare professionals within the Trust.
This role will cover out of hours, predominantly nights, weekend days and nights, and bank holidays.
The post holder will be required to rotate between both sites that make up Calderdale & Huddersfield NHS Foundation Trust.

Develop an understanding of all elements of the nursing process and be able to assist the registered nurse in the ongoing assessment, planning, management, and evaluation of care.
To recognise changes in a patient’s condition which require the intervention of others and understand when to refer to the supervising registered nurse and other senior colleagues for reassessment of patient care.
To work in collaboration with others to ensure the safe and timely discharge of patients with the aim of avoiding readmission.
To perform procedures as detailed within the NMC Standards for proficiency for Nurse Associate (2018) (Annexe B) to meet the nursing needs of the patients in their care.
To work within the NMC code of professional practice, and within policies, procedures, and guidelines of Calderdale & Huddersfield NHS Trust.
To have a willingness to undertake additional skills and training depending on department requirements.
To ensure that high standards of nursing care are given and maintained. To act and raise concerns when standards are not being maintained following local policy and procedures.
To act as an advocate for the patient and the family, including promoting and protecting their interests, privacy and rights, equality, and inclusion.
To be involved in Trust Initiatives to improve patient safety and care.
