Job summary An exciting opportunity has arisen for 2 x Band 6 Nurses fixed term for 12 months to work as a Link Worker with our Psychiatric Liaison Team. The successful candidates will be the responsible Link Workers for the follow up, support and development of an informed plan of care (utilising Dialogue ) for our Service Users who are under the care of the Community Mental Health Teams and who have accessed A&E Services. Main duties of the job The role will be to do preventative work to reduce A&E attendance and to support with contingency plans, signposting, education around what other Services are available and can be accessed as an alternative to A&E and build better links with their CMHT's.
