We are looking for a talented manager to join our integrated management team in West Norfolk. This role provides strategic and operational leadership to our adult health and social care teams and will lead on delivering effective discharge and urgent community response pathways.

The post would be ideal foran experienced health or social caremanager wishing to take on a more strategic role whilst influencing the operational delivery. Experience in leading HR processes and working effectively in a team alongside senior colleagues would be highly advantageous, as would strong budgetary and performance management skills.
Duties include:
· Manage staff within the West Norfolk Home First Hub and urgent community response (UCR)team, including recruitment, performance, professional development and appraisal.
· Lead the improvement of peoples’ experiences of discharge from hospital and across the West Norfolk admission avoidance teams, through redesign of existing services to reduce and eliminate (as far as possible) delays and implement new service developments to promote and embed the home first ethos. This includes establishing and imbedding working with colleagues so that the HFH works effectively as part of the Integrated Urgent and Emergency Care hub for West Norfolk.
· Implement national and local initiatives across health and social care embedding these in operational practice and procedure.
· Develop excellent working relationships with commissioners.

· To provide leadership for West Norfolk Home First Hub for D2A and UCR across West Norfolk to ensure that a high quality, safe and cost-effective service are delivered.
· To identify and explore all opportunities for integration including joint roles where appropriate and to propose options to Operational Director (OD), implementing agreed changes across the HFH.
· To identify opportunities for new business, funding and income and propose to Operational Director. To be responsible for managing and leading performance improvement across the HFH and as required other portfolio services.
· To take an active role in the implementation and delivery of CIP plans, service line reporting and income targets across the portfolio of services.
· To assist and advise the OD on the development and implementation of strategies and policies.
· To develop strong productive relationships with key external agencies including acute providers, independent care home and domiciliary agencies, ICBs, GPs, NHS Norfolk and District Councils.
· Create and build networks to support and contribute to service developments.
