The Individual Commissioning Team is responsible for assessing, commissioning, and case management for individuals with complex health needs.
As a Duty Clinician, you will provide the initial point of contact for the individual commissioning team. You will meet their families and be required to produce a thorough and accurate review of their Fast-Track pathway (face to face), which may then require completion of a Decision Support Tool. You will need to be fully competent in the National Framework for CHC, including Fast-Track and FNC, and operate within local and national requirements.
The role will support the Service Manager with ensuring that statutory responsibilities are met, which may include support with the Duty Triage process if required.

Service operating hours are Monday to Friday 09:00 – 17:00.
You must possess a current NMC (Adult, Mental Health or LD posts available) professional registration and have the ability to travel within the locality.
You will be a competent professional with clinical experience within your field of practice and have a full understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care, Who Pays guidance, and relevant sections of the Mental Health Act and commissioning and contracting principles.
Ideally, you will be an experienced CHC assessor; however, if you are an experienced Nurse, England registered professional, and do not have CHC experience, we would still like to hear from you.
1. The Duty Clinician will coordinate all hospital discharge referrals, CHC fast track, and all urgent matters where the allocated case manager is unavailable. They will work as a daily point of contact for all matters requiring an urgent response.
2. The postholder will have a flexible approach to care and will work across inter-agency boundaries, maintaining good communication and liaison skills with all professionals and members of the public.
3. The postholder will lead the commissioning and management of packages of care for people in a variety of care and support settings.
Communication & Relationships Skills
1. To provide relevant and timely specialist advice and guidance on functional and informational matters.
2. To support the development of internal and external communications where required by regular contact with the teams, stakeholders, and Communications team.
3. To be responsible for the preparation of correspondence and complex reports, assessments, and papers.
4. To undertake complex clinical assessment and clinical report writing.
5. To represent the team on panels and decision-making forums as required, participating in the decision-making process and clarification of funding responsibilities for clients who are registered with an NHS Shropshire, Telford and Wrekin GP.
6. To represent NHS Shropshire, Telford and Wrekin on NHSE appeal panels.
7. To work with team members and other health and social care professionals to promote joint working to achieve positive benefits and added value for individual service users and carers.
8. To liaise and communicate with external agencies including NHS England, care providers, and solicitors.
9. To maintain constructive relationships with a broad range of internal and external stakeholders.
10. To participate in relevant internal and external working groups/projects, services, and initiatives which may be highly complex, sensitive, political, and contain contentious information with the aim of providing information and analytical advice to the teams.

1. To undertake complex and detailed analysis of specific projects and patients requiring high levels of concentration.
2. To update, maintain, organise, gather, and analyse information to predict/meet future individual patient, organisational, and team needs by identifying best professional practice.

Planning & Organisational Skills
1. To plan own workload and diary to ensure local, national, and statutory targets and deadlines are met.
2. To undertake complex clinical assessment and care planning to ensure patient needs are met and that where appropriate, step-down/discharge is planned and coordinated.
3. To work with members of the team and key stakeholders to investigate the causes of any variance from plan/delivery targets and contribute to the implementation of solutions.
4. To manage a defined caseload undertaking responsibilities as delegated by the Team Leader/Manager, including routine management of complex cases and the planning, organising, and commissioning of specialised care packages ensuring the maintenance of accurate records.

1. To contribute to the strategic planning recommending adjustments as necessary.
2. To support the implementation of strategic modernisation/service improvement, public health, workforce or commissioning strategies and any associated action plans, in accordance with the agreed priorities of the team.
3. To contribute to the development of performance and governance strategies and the implementation of improvement programmes.

1. To provide specialist training, advice, and support on own role/responsibilities across NHS Shropshire, Telford and Wrekin and the wider system.

2. To maintain accurate and comprehensive clinical records.
