An excellent opportunity has arisen for an enthusiastic, qualified, experienced, Lead Nurse with the ability to work well under pressure to join our friendly practice team with 5 partners and 8,400 patients.
Main duties of the job
The Lead Nurse will be responsible for several clinical areas such as infection prevention and control, health promotion, chronic disease management, health prevention, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
The Lead Nurse will also be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients. They will also ensure nursing services are delivered effectively to the entitled patient population.

Job description Job responsibilities
The following are the core responsibilities of the Lead Nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
1. To produce the IPC annual statement in conjunction with the Practice Manager
2. To act as the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing audits, training, support and evidence of compliance to the Practice Manager
3. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice
4. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
5. To develop, implement and embed health promotion and wellbeing programmes
6. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
7. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health
8. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
9. To ensure compliance with the practice chaperone policy
10. To ensure SNOMED CT codes are used effectively by all members of the nursing team
11. To ensure the appropriate use of chronic disease registers
12. To co-ordinate the provision of travel medicine services
13. To implement vaccination programmes for adults and children
14. To support the clinical team with all safeguarding matters, in accordance with local and national policies
15. To deliver opportunistic health promotion where appropriate
16. Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
17. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
18. Refer patients directly to other services/agencies as appropriate
19. Recognise and work within own competence as regulated by the NMC
20. Processing of administration in a timely manner, including referrals and other associated administrative tasks
21. Maintain accurate clinical records in conjunction with good practice, policy and guidance
22. Working collaboratively, accepting an equal share of the practice workload
23. Adhere to best practice recommended through clinical guidelines and the audit process
24. To request pathology services and process pathology results as required
25. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
26. Contribute to the successful implementation of continuous improvement and quality initiatives within the practice
27. Attend and contribute effectively to practice meetings as required
28. Prepare and complete the revalidation process
29. Participating in an annual individual performance review taking responsibility for maintaining a record of own personal and professional development.
30. Commit to self-learning and instil an ethos of continuing professional development across the practice team
31. Support the training of medical students from all clinical disciplines
32. Support the partners in achieving the strategic aims of the practice, making recommendations to enhance income and reduce expenditure
33. Review and adhere to practice protocols and policies at all times
34. Promote Sepsis awareness within the practice
35. Encourage collaborative working, liaising with all staff regularly, promoting a culture of continuous improvement at all times
36. Overall management of the nursing team, providing guidance when necessary, acting as a mentor to assist, maintain and develop clinical skills
37. Maintain an effective nursing staff rota, ensuring all clinics are staffed appropriately
38. Take and test pathology samples from patients for analysis
39. Other chronic disease management/QOF activities
40. Support and advise women with family planning needs, including contraception and safe sex advice
41. Maintain professional registration and adhere to revalidation requirements
42. Perform other clinical duties as required, within the scope of their competencies and training
43. Carry out, support and advise women with regards to cervical smear tests
44. Care for patients with uncomplicated wounds, including (but not limited to):
45. Removal of sutures
46. Tissue viability assessment
47. Wound cleaning and dressing
48. Management of burns and scalds
49. Management of skin conditions and referral where necessary
50. Implement and participate in vaccination and immunisation programmes for both adults and children, including (but not limited to):
51. Routine immunisations
52. Pertussis immunisation for pregnant women

54. Pneumococcal immunisation

Person Specification
Qualifications
Essential:
* Registered nurse (Nursing and Midwifery Council)
Desirable:
* Post graduate diploma or degree (Chronic Disease Management)
* Nurse prescriber
* Mentor or teaching qualification
* Completed or working towards formal infection prevention and control training
Experience
Essential:
* Experience of working in a primary care environment
* Experience of leading a nursing team in a primary care environment
* Experience of working as a practice nurse or community nurse
* Experience of chronic disease management
* Experience of working autonomously
* Experience of infection prevention and control measures
