An exciting opportunity has arisen for a Band 7 Macmillan Clinical Nurse Specialist post within the Hull and East Riding Community Specialist Palliative Care Team.
This multi-disciplinary team delivers specialist, high quality care to patients with advanced, progressive illness and their families and carers. As an RN with a minimum of 2 years working within a palliative care setting you must have advanced palliative care clinical practice experience, knowledge and skills.
Applicants must have excellent, advanced communication skills and experience of multi-professional team working. They must also have a commitment to the delivery of high quality patient care. You must be qualified to degree level and have extended Non-Medical Prescribing qualification.
Main duties of the job
The post holder will work as part of the wider Specialist Palliative Care Team and be responsible for providing complex / specialist care, advice and support to patients with a life limiting illness who are experiencing symptoms that require specialist care.
The post holder will be required to manage a caseload of complex patients independently and ensure full participation in the MDT to ensure safe care and service delivery. It is essential that the post holder be a confident and competent practitioner who will recognise and engage in service development and progression.
The post holder will be expected to travel within the geographical boundaries of the East Riding of Yorkshire. The main geographical location for this post will be within the Holderness locality with the flexibility to cover other areas as part of the wider Specialist Palliative Care Team.
This post is fixed term for 12 months. There will be an opportunity to review this prior to the end date and the potential to extend for a further 6 months.
