The Tarentfort Centre is a 20 bedded rehabilitation inpatient unit catering for men from the age of 18. It comprises of two clinical areas: an acute and sub-acute learning disability forensic.
We specialise in the assessment and treatment of men with a learning disability whose offending behaviour and complex mental health needs require care in a low secure setting. This can be a challenging yet rewarding environment and requires excellent organisation skills and the ability to use your initiative. The Healthcare Worker is a key member of the multidisciplinary care team and will have continuing responsibility as follows:
* Under limited supervision from a senior associate worker, qualified nurse, occupational therapist or the nurse in charge, deliver an effective and competent level of care, which is determined by the patients' care plan.
* Supervise the care given by band 2 healthcare workers.
* Be fully aware of the needs of patients over a 24hr span of care.
Main duties of the job
* Support the work of the multidisciplinary team.
* Consistently deliver a patient-focused service, which promotes optimum independence.
* Offer appropriate support and advice to the patients.
* Record accurately all care given as required.
* Enable patients to maintain and improve their independence by promoting active involvement in the therapeutic activity programme.
* Note and report accurately any care given and/or information required by the nurse in charge.
* Give physical care as required or instructed.
* Promote patients' appearance and personal hygiene by offering support/advice in self-care and ensuring that suitable toiletries are available.
* Promote a healthy, safe and secure environment by adhering to Trust statutory health and safety requirements and maintaining confidentiality of information at all times in compliance with the Trust Policies.
* Assist patients to maintain a clean and healthy ward environment and assist them in all aspects of daily living skills as required.

Job responsibilities
* Record and report accurately any changes related to the patients' current behaviour and/or risks to Nurse/healthcare professional in Charge in a timely manner.
* Record care given and report omissions accurately and in a timely manner.
* Support and carry out therapeutic observations as required and instructed.
* Make recommendations to the care plan process.
* Promote a healthy, safe and secure environment by adhering to Trust statutory health and safety requirements and maintaining confidentiality of information at all times in compliance with Trust policies, procedures, protocols and guidance.
* Act as a role model and support new team members through demonstration and explanation of tasks and techniques.
* Facilitate the assessment of staff undergoing the Care Certificate.
* Demonstrate a professional approach to work, and abide by the Trust values.
* Provide comfort, compassion, empathy and time to patients and their carers, family and loved ones.
* Undertake and maintain all essential training to role.
Person Specification
Experience
* Experience of working within Mental Health/Learning Disabilities setting.
* NHS Experience.
* Experience of Forensic setting.
Knowledge and Skills
* Have a good understanding of mental health conditions appropriate to the setting.
* Numeracy and Literacy Skills.
* Able to communicate effectively with a wide range of people.
* Ability to work in a multi-disciplinary team.
* Working knowledge of relevant policies and procedures.
Training and Qualifications
* Health or Social Care Qualification (Level 3 or equivalent).
* Computer Literate.
