The Home First Intermediate Care Service helps patients achieve independence by facilitating earlier hospital discharges and avoiding hospital admissions. You will assist in delivering high-quality healthcare and support patients in their homes by following prescribed care and therapy packages.
The service aims to provide outstanding, patient-focused, community-based healthcare and therapy, supporting patients to reach their goals through early discharge, assessment, and admission avoidance.
Our operating hours are from 7am to 10pm, seven days a week.
Please note: This post may be offered as a Band 2 Development post depending on qualifications and experience, with full support for development into a Band 3.
Requirements: You must be able to travel independently around the county, meeting strict deadlines. A full UK driving license and access to a vehicle are essential. Public transport is not suitable for this role. Please confirm in your application that you meet these criteria.
Main Duties
1. With training, use skills from Nursing, Allied Health Professionals, and Medicines Management to undertake delegated tasks.
2. Apply competency-based treatment techniques under supervision, following prescribed plans.
3. Provide care to patients experiencing health deterioration, focusing on avoiding hospital admission.
4. Deliver functional assessments, personal care, medication prompts, mobility exercises, and equipment provision.
5. Carry out duties delegated and supervised by registered professionals.
6. Maintain a high standard of care.
7. Manage your own caseload of patients with complex needs requiring ongoing support.
