We have an exciting opportunity for an experienced and innovative Advanced Nurse Practitioner to join the team of Nurse Leaders within the Central Integrated Health Community.
The role is to provide operational, managerial, and professional team support to Advanced Nurse Practitioners, trainee ANPs, and Nurse Practitioners working across our community resource teams in the central IHC. They will foster a culture of excellence and continuous improvement, ensuring the highest standard of nursing care is provided, monitored, and evaluated within the ANP teams.
Main duties of the job
Key aspects of the role will include:
1. Working as an autonomous, advanced practitioner, with an extended scope of practice to work with the team to deliver care providing a high standard of specialist assessment and treatment of patients. Using evidence-based principles/current best practice and advanced specialist skills and clinical reasoning to assess, plan, implement, and evaluate interventions.
2. Ensuring all clinical practice is based on current research/evidence, developing and undertaking audit and research as appropriate. Participate in quality improvement initiatives, clinical audits, and research projects to enhance patient outcomes.
3. Demonstrating through day-to-day action a contemporary approach to Primary & Community Care which is patient-centred, prudent, and team-enabling.
4. Maintaining a visible professional presence where patients, carers, and staff can turn for assistance, advice, and support.
5. Leading and participating in projects and areas of work, which will reflect clinical, professional, and corporate objectives.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
