Discharge to Access Community Mental Health Nurse

We have a unique opportunity for a Community Mental Health Nurse to bring their expertise into the teams that support Hertfordshire residents. This opportunity allows professional development enhancing physical health skills, frailty and the impact this has on mental health.
* Discharge to Access (DTA) is a national model of care to ensure safe discharge from acute hospital settings, continuing care, and assessment out of hospital. The service supports people to have an extended period of assessment of longer-term needs in the right place. This is a 28-day pathway to provide multidisciplinary support to individuals in a DTA bed in a care home setting.
* You will also have the opportunity to enhance your knowledge and skills in physical health and social care. As a 'wrap around' service, you will be linked in with various health and social care professionals to deliver a person-centered approach to the service user.
* Candidates must be a car driver and have a mode of transport (unless you have a disability as defined by the Equality Act 2010 which prevents you from driving) as a high degree of travel is required. Work will be undertaken in the office base and in care homes around the county.

Main Duties of the Job
* Provide mental health assessment and risk assessment to people in a Discharge to Assess bed and provide support, advice and guidance to the Discharge to Assess MDT and care home.
* Liaison with various professionals, family, carers, and care home staff.
* Documentation and information gathering.
* Identify conditions affecting older people and aging.
* Coordination and liaison of appointments with service users, MDTs, and care providers.
* Attend MDTs - identifying appropriate referrals.
* Robust communication skills.
* Liaison with secondary mental health services when required.
* Compassionate and sensitive engagement with service users and families/carers.

Key Relationship
The post holder will be working within the community most of the time and will need to establish and maintain positive relationships with people, agencies, charities, and other providers who also provide services to Older People in that community area. These relationships are vital, and team members should bear in mind that they will be representing HPFT. Close working relationships should be developed with other parts of HPFT notably Crisis teams and MHSOPs; as well as Hertfordshire County Council's Health and Community Services.

Job Responsibilities

Clinical Responsibility
* Carry out specialist mental health and social care assessments. Develop, implement, evaluate, and document specialist care.
* Devise care plans that promote patients' independent living in the community if possible.
* Complete risk assessments that are clear, concise and detail plans to safely manage any risks that are identified.
* Conduct physical health checks.
* Attend and represent/feedback when required at MDTs.
* Review clinical notes on patient records.
* Support discharge planning with referral to appropriate services or signposting.
* Demonstrate a high standard of record keeping and documentation with adherence to Trust policies.
* Work with colleagues to develop and demonstrate clinical expertise and good standards of mental health practice.
* Deliver care that is person-centered reflecting current best practice, and challenge practices that may be detrimental to older people receiving high quality care.
Service Development and Improvement
* Ensure that all policies applicable to the service are implemented.
* Propose ideas for, and if accepted, complete audits relevant to the improvement of the team's clinical practice.
Communications
* The post holder will have a good level of competency to receive and give clinical information verbally and in writing from/to those receiving care, their families/carers and professional colleagues.
* Demonstrate a good level of interpersonal skills, self-awareness and empathy in all communications.
* Ensure adherence to all aspects of patient confidentiality, documentation and record keeping according to Trust and national guidelines.

Person Specification
Qualifications
* Degree, diploma or equivalent in Nursing (Mental Health Nursing).
* Registration with Nursing and Midwifery Council.
* Practice Assessor course completed.
* Good experience in older people's mental health, frailty and community.
* Pre or Post registration training/ short courses in mental health.
* Community and Inpatient nursing experience.
* Physical health training.
* Experience of working with clients with Delirium.
* Experience of working in primary care.
Experience & Knowledge
* Knowledge of legislation related to mental health and social care (e.g. Mental Health Act, Mental Capacity Act, Care Act etc)
* Good knowledge and skills in Microsoft Word, Excel, PowerPoint.
* Ability to manage distressing and challenging situations. De-escalation skills.
* Competent in the use of IT systems Paris and SystmOne.
