Clinical Specialist (Single Point of Access)

Single Point of Access is a multi-professional team providing advice surrounding patient management. We are looking for an experienced clinician (Nurse, Therapist or Paramedic) with extensive knowledge of community services to join this care coordination team. The remit of this role will be to provide assessment of a patient's needs, and advice regarding service availability in order to keep this patient in their own home and avoid unnecessary admissions to the acute hospital setting. This post will be able to actively review cases in the SWAST call stack to review where vital emergency services can be spared, and support patients to remain well in their own homes. It will include liaising with various community teams to ensure timely assessment and management of patients in their own homes.

Job responsibilities
We require a forward-thinking, dynamic, motivated and creative individual to join us. Being able to work as part of a team and under pressure are essential skills that the successful candidate will hold. Experience and understanding of the Continuing Healthcare Framework, Mental Capacity Act and Safeguarding are also essential, along with in-depth working knowledge of community services and pathways.
This role is a vital link between acute and community services, to provide seamless care and advice to optimise the patient's journey. This service is evolving and we are looking for someone to be involved in helping us shape the services for the future.

This is a fixed term contract for 6 months. For current NHS employees this post could be a secondment opportunity.
