Job summary
To work as a member of the Rapid Response & Intermediate Care Team in (the community) in Buckingham and surrounding villages providing nursing, social and rehabilitative care which may include some nursing, physiotherapy and occupational therapy-based interventions for designated patients (either individuals or a specific clinical group). The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission into the Acute Service and facilitate timely discharge into the community.
A full driving licence with business insurance and access to a car for work purposes.
Main duties of the job
1. Providing general and specific care to adults within competencies
2. Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of care including nursing, social and therapy based rehabilitative care to patients in liaison with senior staff.
3. Taking responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients' own homes.
4. Being responsible for the completion and potential delegation of non-clinical tasks within the team.
5. Improve the care and support of patients in the community, so patients can remain safely in their own home for as long as possible
6. Have an innovative approach, excellent communication skills and willingness to work with other members of the community teams to ensure high quality service delivery
7. Willingness to further develop your clinical expertise in a range of areas.
