Job Summary:
To deliver nursing care in accordance with national and local policy and guidance and in accordance with the Nursing and Midwifery Council.
The post holder will be a Registered Nurse in the community, known as a ‘community nurse,’ who works as part of a team to deliver nursing care and treatment autonomously as delegated by and under the direction of the District Nurse.
Job Purpose:
1. The post holder will visit adult patients in their own home who have an identified nursing need, including those with a long-term condition, complex care needs, and those needing care at home and at the end of life.
2. The post holder will use an electronic patient-held record system, EMIS, as a record of care. All nurses are provided with a work iPhone and iPad for mobile working.
3. The community nurse will utilize a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care, and symptom control. Upon successfully attaining all required competencies, they will be reviewed for a Band 6 Senior Community Nurse post.

All community nurses work as part of a local team, closely with the local acute hospital, social care, mental health teams, GPs, and Practice Nurses using the electronic clinical record, EMIS, with a work iPad and iPhone provided. #J-18808-Ljbffr
