Job summary An excellent opportunity has arisen for Ash Trees Surgery to recruit a diabetic specialist nurse, who will support our current team. The post holder will be a highly experienced Practice Nurse/Long-term conditions nurse and have a good knowledge of monitoring people with diabetes and titrating medications as required. Working with the medication management team and GPs to change medications as needed. In this role you will be working as part of multi-disciplinary team to ensure that all sites are working consistently towards QoF targets, whilst reaching the desired timescales. As an autonomous practitioner you will be able to demonstrate critical thinking and decision-making relating to long term conditions. Main duties of the job Direct clinical involvement in monitoring and management of diabetes. Delivery of nursing care to patients in the practice population.
