The post holder will be the clinical lead for the UCR function of the Home First Service for SDHC.

Qualifications
Essential criteria
1. Registered first level nurse/practitioner with relevant health degree
2. Completed Master's Degree in Advanced Practice or working towards
3. Non-medical independent prescribing for healthcare professionals or working towards
4. Advanced clinical practice skills/competencies
5. Management of patients with acute medical illness
6. Clinical examination skills
7. In-depth knowledge in their specialist clinical field
Desirable criteria
1. Demonstration of Leadership or management experience
Experience
Essential criteria
1. Significant post registration experience
2. Leadership in quality initiatives such as clinical benchmarking, service development
3. Significant experience in audit and research
4. Healthcare professional prescribing
Skills
Essential criteria
1. Clinical leadership skills
2. Communication skills, both written and verbal
3. Negotiation and conflict management skills
4. Change management
5. Car driver with current UK driving license and access to car for work
